استخدام البنسليناز مقاومة البنسلين ارتبط السلبية الدموية من الآثار بما في ذلك قلة العدلات, الكريات, المحببات وقلة الصفيحات, خاصة عندما تعطى في حقنا جرعات. ندرة المحببات و نزيف لفترة طويلة من الوقت وقد تم الإبلاغ عن نادرا. العلاج مع البنسليناز مقاومة البنسلين ينبغي أن تدار بحذر في المرضى الذين يعانون مسبقا اعتلالات الدم أو نخاع العظام والاكتئاب وظيفة المكونة للدم ينبغي رصدها. تعداد الدم مع التفاضلية ينبغي أن تتم قبل بدء العلاج و 1 إلى 3 مرات أسبوعيا أثناء العلاج. الدموية تشوهات عموما عكسها و حل في غضون عدة أيام إلى أسبوعين بعد التوقف عن العلاج.
