ضعف فعالية ممكنة تأخر إفراز حلقة مدرات البول قد تحدث في المرضى الذين يعانون من الفشل الكلوي. هؤلاء الأفراد قد تتطلب جرعات عالية التي ترتبط مع زيادة خطر تشوهات بالكهرباء (نقص صوديوم الدم, نقص الكلور قلاء, البوتاسيوم في الدم, نقص مغنيزيوم الدم, نقص كلس الدم) و مؤقت (طنين الإذن, فقدان السمع). العلاج مع مدرات البول حلقة ينبغي أن تدار بحذر في المرضى الذين يعانون بشكل كبير من ضعف في وظيفة الكلي. إطالة أمد الجرعات على فترات قد تكون مناسبة لمنع تراكم المخدرات. يجب مراقبة المريض عن كثب بحثا عن علامات وأعراض السوائل أو عدم التوازن المنحل بالكهرباء ، بما في ذلك جفاف الفم, العطش, الضعف, الخمول, نعاس, الأرق, آلام العضلات أو تقلصات العضلات التعب, انخفاض ضغط الدم, قلة البول, عدم انتظام دقات القلب, عدم انتظام ضربات القلب أو اضطرابات الجهاز الهضمي مثل الغثيان والقيء. الإفراط في إدرار البول يجب تجنبها لأنها قد تحدث الجفاف و نقص حجم الدم الذي يمكن أن يؤدي إلى انخفاض مفاجئ في الترشيح الكبيبي الكلوي تدفق الدم. إذا زيادة آزوتيمية و قلة البول تحدث أثناء العلاج من المرض الكلوي المترقي ، أو إذا كانت وظيفة الكلى يصبح أسوأ تدريجيا كما أشارت إلى ارتفاع كعكة أو ارتفاع مستوى الكرياتينين في الدم, انقطاع أو التوقف عن العلاج يجب اعتبار.
